Indication:IgA - Confirmatory test following borderline/positive result for tissue transglutaminase (tTG) antibodies
IgG - Performed in suspected or known IgA deficient patients.

Background: EMA recognise tissue transglutaminase (tTG) and are used to confirm a positive tTG result. May be negative in children < 2 years old with coeliac disease, in whom IgG gliadin antibodies will be measured. IgA EMA are positive in > 90% of untreated adults with coeliac disease and in 60 - 70% of dermatitis herpetiformis patients. Rare in other enteropathies. Important in investigation of paediatric malabsorption. The antibodies disappear on gluten-free diet and thus may be used to monitor adherence to the diet. IgA EMA are absent in patients with IgA deficiency, who have an increased incidence of coeliac disease.
